We are working with a state government client who is seeking an experienced Senior Support Planner (AO6)
to join their team. This role plays a critical part in supporting participants, their families and carers within a complex and evolving scheme environment.
About the Role
This role will work closely with participants and internal stakeholders to assess needs, develop support plans, and contribute to continuous service improvement. Key responsibilities include:
- Providing participants, families and carers with information and support about the Scheme and the Agency.
- Assessing participant needs and creating or amending support plans, including making decisions or recommendations regarding necessary and reasonable treatment, care and support.
- Advising and supporting less senior team members by providing technical guidance and sharing knowledge.
- Discharging delegated functions in accordance with the relevant legislation.
- Empowering participants in the development, implementation and monitoring of support plans that reflect their goals, choices and measurable outcomes.
- Identifying effective treatment, care and support options that balance participant needs with Scheme sustainability and community expectations.
- Liaising with legal representatives of participants and their families/carers as needed.
- Supporting participants to use tools and resources to maximise independence, community participation and employment.
- Collaborating with stakeholders to ensure successful implementation and maintenance of support plans.
- Assisting in service improvement initiatives, including resource development under guidance from senior staff.
- Actively contributing to a learning culture by sharing knowledge and staying current with best practices.
About You
- Strong knowledge and practical understanding of support planning principles and processes.
- Proven experience managing complex support planning and goal-setting processes, including discharging statutory functions.
- Well-developed understanding of insurance management principles and personal injury claim management for catastrophically injured individuals.
- High-level organisational skills and the ability to lead self and others in a dynamic, agile environment.
- Excellent communication skills with the ability to build rapport, convey complex information, negotiate effectively, and mentor team members.
- Sound decision-making skills based on evidence, particularly in relation to disability services.
- Responsiveness and flexibility in addressing participant and family/carer needs.
The Benefits
- $79.17 per hour + Super (PAYG Agency Contractor)
- Contract until June 2026
